1095 Portal: Receiving Your Form 1095
Stanford regular employees may log into My Benefits to download, print and update IRS Form 1095 delivery preferences. The IRS Form 1095 will be sent by U.S. Mail unless you elect your electronic delivery by the first week of January.
Postdoctoral scholars, former employees and other covered individuals will receive their Forms via U.S. mail, even if electronic delivery was previously elected. Stanford University has disabled direct access to the 1095 Portal as the login process requires use of personal identifiable information (PII) and considers it a security risk.
The IRS Form 1095 for each tax year will be produced by March of the next year, and employees who elected electronic delivery will be sent an email when it is available for download. To access your form or update your delivery preferences for the future:
- Log into My Benefits
- Click "Main Menu" at the top of the page
- Under "Items to Explore," click "ACA 1095 Portal"
- Click “My Form 1095” tab
What is the IRS Form 1095?
The Affordable Care Act (ACA) requires that you have medical coverage and that you can provide proof of coverage. As a result, you may receive an IRS Form 1095 in January to indicate if you and any dependents were offered and/or enrolled in medical coverage through Stanford University. Depending on how you obtained your medical coverage, you may receive a 1095-B, a 1095-C, or even both.
