1095 Portal: Receiving Your Form 1095

Employees may log into My Benefits to download, print and update IRS Form 1095 delivery preferences. The IRS Form 1095 will be sent by U.S. Mail, unless you updated your delivery preference to electronic delivery by the first week of January. Postdoctoral scholars and past employees will receive their Form via U.S. mail, even if you have previously selected electronic delivery. Stanford University has disabled direct access to the 1095 Portal as the login process requires use of personal identifiable information (PII) and considers it a security risk.

The IRS Form 1095 for each tax year will be produced by January 31 of the next year, and you will be sent an email when it is available for download. To access your form and update your delivery preferences for the future:

  1. Simply log into My Benefits
  2. Click "Affordable Care Act 1095 Tax Form" from the "How Can We Help" tab then "Form 1095 Delivery Preference"
  3. Click on “Health Coverage Form 1095” to download your form and/or follow the directions to change your future delivery preferences

What is the IRS Form 1095?

The Affordable Care Act (ACA) requires that you have medical coverage and that you can provide proof of coverage. As a result, you may receive an IRS Form 1095 in January to indicate if you and any dependents were offered and/or enrolled in medical coverage through Stanford University.  Depending on how you obtained your medical coverage, you may receive a 1095-B, a 1095-C, or even both.
