Requesting Colleague Feedback Job Aid for Managers

Welcome to the Grow and Perform at Stanford performance program.
This goal-setting job aid aims to assist you through the feedback request process step-by-step. All steps for you to complete will be purple, and all notes will be highlighted in teal.
Launching GPS
- Navigate to Axess and login using your SuNET ID.
- Select 'Employee Center' in the navigation at the top of the webpage.
- Scroll down to the Performance Management menu in the right-hand column of the webpage and select Grow and Perform at Stanford (GPS) System.
Requesting Feedback from Colleagues
Step 1: Select Career and Performance under the My Team tab.
Step 2: Select Evaluate Performance.
Step 3: Select the No Participants hyperlink.
Step 4: Select the +Add button.
Step 5: Enter and select the employee's name to request feedback.
Step 6: Select Send Request.
Step 7: Select a due date for the feedback by selecting the calendar icon.
Step 8: Select Submit.
Note: The following questions are included in the feedback form sent to participants. Completion of all four questions are not required.The foru questions are as follows:
- Provide at least one example of how the employee has accomplished their essential job functions.
- List at least one area of opportunity for the employee’s improvement within their practice and professional growth.
- Give at least one example where the employee has demonstrated a Core competency and/or value.
- Give at least one example of how the employee has provided a positive experience in their area.

- For system-related issues, submit a GPS help ticket.
