Report deaths or serious injuries at work immediately
In compliance with Cal/OSHA regulations, employers are required to report workplace deaths or serious injuries within 8 hours, including those that occur during off-site work, such as fieldwork or work travel.
Cal/OSHA defines an injury as serious if:
- It requires inpatient hospitalization for reasons other than medical observation or diagnostic testing
- An employee suffers a loss of an eye or any limb of the body
- An employee suffers any serious degree of permanent disfigurement
If you become aware of a death or serious injury at work, you must immediately contact the Environmental Health & Safety 24-hour hotline at 650-725-9999.
Please note that Cal/OSHA imposes fines for delayed reporting, and all penalties will be paid by the local department.
