Vaccination Requirements for Employees in Health Care Facilities

Stanford employees who work in health care facilities must comply with the Stanford Medicine vaccination requirements. Per the Stanford Medicine vaccination requirements, all clinical, patient-facing employees (faculty, staff, postdoctoral scholars, student employees) at the School of Medicine, including those who work in or frequent clinical and hospital areas, must be up to date with COVID-19 vaccination (completion of primary vaccine series) in alignment with Centers for Medicare and Medicaid Services protocols.
Stanford employees who do not frequent a hospital or clinical building are not required to comply with this vaccine requirement.
To comply with this requirement, faculty, staff and postdoctoral scholars should ensure they have completed the primary series of the COVID-19 vaccination.
- Faculty and clinical educators on the medical staff must submit their vaccination and booster information through Stanford Medicine's COVID-19 Proof of Vaccination form.
- Faculty, clinical educators, staff, and postdoctoral scholars must submit their vaccination information through Health Check.
Those who wish to submit a medical or religious accommodation for the vaccination requirement should follow the accommodation process.
New Hires
New hires must comply with this vaccine requirement and submit proof of vaccination to Stanford Medicine Workforce Health and Wellness at 300 Pasteur Drive, Ground Floor, H0124 or via email to whw@stanfordhealthcare.org.
If you are a new hire requesting an accommodation, please submit one medical or religious accommodation. Please note that new hires must provide proof of vaccination or have a medical or religious accommodation approved by the university in advance of their start date.

Faculty, staff, and postdoctoral scholars may apply for an accommodation based on a disability or medical condition. Medical accommodations will be allowed only for specific reasons consistent with guidance from the Centers for Disease Control and Prevention (CDC), including:
- Documented history of severe allergic reaction to one or more components of all three currently approved COVID-19 vaccines.
- Documented history of severe or immediate-type hypersensitivity allergic reaction to a COVID-19 vaccine, and separate contraindication to other available formulations.
