Program Participants

To meet the requirements of the Protection of Minors policy, and safeguard the health and wellness of participants in the program or activity, all Program Participants must complete:

1. A liability waiver
2. A medical information form

For programs that occurred before the launch of the Protection of Minors Management System (ePOM) on April 6, 2020, liability waivers should be retained until two years after the Program Participants turn 18 years old.
