VSP Retiree Vision Plan Resources

Retiree vision care benefits are provided by Vision Service Provider (VSP) using their Choice Network of providers. You may go to any vision care provider, but when you use a VSP provider, you make a copay at the time of service and do not have to file a claim. 

Plan Documents

A Summary Plan Description (SPD) or Evidence of Coverage (EOC) is the official, detailed plan document for each plan outlining information about eligibility; costs and cost-sharing; included and excluded services; claims process; procedures for filing grievances, complaints, and appeals.
