When making your own long term plans as you age, or caring for someone you love, no one needs to go it alone. Session speakers will address best practices for developing trustworthy teams of personal and professional support most needed during times of transitions to higher levels of care, during an acute crisis, during times of both grief and joy.
Paula Wolfson, LCSW, Manager of Avenidas Care Partners, will review a contingency plan template to help you visualize coordinating necessary legal, health and financial teams. Participants will also hear from a licensed fiduciary to understand the role they can take as a ”team leader," coordinating a client’s personal and professional support team, for those who need assistance managing parent’s or a loved one’s illness, or one’s own.
